Posthumous honors were granted to Elvis Presley, Babe Ruth and Antonin Scalia, the conservative Supreme Court justice.
The other recipients include doctor and philanthropist Miriam Adelson, Republican Sen.
Trump's homage to Elvis - who once met with president Richard Nixon at the White House in 1970 - paused for a recording of the gospel song "How great thou art".
The Presidential Medal of Freedom is the nation's highest civilian honor. After listing their nine children by name, Trump commented: "You were very busy".
"Universally admired for his towering intellect, brilliant wit and fierce devotion to our founding principles, Justice Scalia has made a deep and lasting impact on the history of our nation", Trump said. "I always knew I liked him", Trump said, sparking some laughter from the audience.

"It helps. That's the way it is. I guess I'm not supposed to say it, but that's the way life works, right?"
Scalia was an Associate Justice of the Supreme Court of the United States from 1986 until his death in 2016 and was appointed to the Court by President Ronald Reagan in 1986.
What happened next? Well, a good portion of the Twitterverse is convinced Trump wasn't just admiring Scalia for having a big family, but he was also impressed that Scalia and his wife, um, consummated their marriage so many times. A physician, she he has practiced internal and emergency medicine, specialized in the disease of narcotic addiction, and founded two research centers committed to fighting substance abuse, the White House said. She has also supported Jewish schools, Holocaust memorial organizations, and Israeli military personnel and veterans, the White House noted. They donated a combined $50 million to the Congressional Leadership Fund, which spent more than $137 million against House Democrats but lost a majority of races it focused on during the general election.
Honoring donors is a move that's been criticized by both sides of the aisle.
Ruth's grandson, Thomas Stevens, accepted the award for the late baseball player who's still considered one of the greatest of all time.
